Responsibilities

The American Heart Association has a phenomenal opportunity for a Community Health Manager. The Community Health Manager will implement and perform the duties outlined in the United Healthcare Foundation grant for Montgomery County, Maryland, including the management of the clinical and operational metrics related to the Stop the Silent Killer Blood Pressure initiative working to control hypertension rates in three clinical settings.

  • This is a grant-funded, 2-year temporary (22.5/hours per week) position.

Crucial job duties:

  • Will lead all aspects of practice assessment processes for up to 11 Health Care Organizations (HCOs) through relationship management in collaboration with regional staff and leadership.
  • Responsible for daily operations related to grant measurables with strong attention to detail.
  • Prioritize, plan, and implement encouraging activities including, but not limited to regular communications to cohort, sharing of resources, and assisting with CME and non-CME credit bearing events.
  • Focus on hypertension and clinical operations improvement, implementing policy of systems changes.
  • Engage cohort HCOs to ensure standard processes and quality improvement for treatment of patients through the American Heart Association’s outpatient program: Target:BP

Qualifications

Minimum qualifications:

  • 2+ years of relevant clinical experience
  • Some medical/nursing/STNA training/education/certification preferred
  • Experience in outpatient/ambulatory clinical setting, practice facilitation, and/or work with electronic medical records, clinical studies or quality improvement programs is helpful
  • Account management and project management experience is preferable
  • Ability to engage medical professionals at all levels and C-suite professionals
  • Presentation and event-management experience useful
  • Knowledge of federally qualified health clinics helpful
  • Food security knowledge useful
  • Culturally and medically astute interpersonal communication skills
  • Ability to work independently and provide own transportation to AHA Arlington Office and three area clinics.
